If you are someone looking to grow in the healthcare field , this is a great opportunity. This Care Coordinator will be traveling within the field in Elgin IL and will work remotely. You do not need to be licensed for this position. Please apply and we will reach outJob Description: Perform duties related to the day-to-day operations of the High Risk Case Management functions to include working with members identified as high risk to identify needs and goals to achieve empowerment and improved quality of life.Work telephonically and in the fieldwith patients identified as high risk to identify needs, set goals and implement action steps towards achieving goals. Empower patients to help them improve their quality of life.Comply with established referral, pre-certification and authorization policies, procedures and processes by related Medical Management staff.Participate in on-going communication between case management staff, utilization management staff, health plan partners and contracted providers.Travel to different Members Homes, and or skilled nursing facilities and long-term care facilities to perform member assessments and treatment planning.Assist with the implementation of policies and procedures regarding case management and utilization management functions.Maintain compliance with federal and state regulations and contractual agreements.Coordinate case management functions with other departmental functions as assigned.Conduct appropriate knowledge/education and interventions for members defined to be at risk.Requirements:5+ years working with the MI, DD, Aging, Medicaid Waiver, or HIV/AIDS populations.Do NOT have to be licensed for this position, but if you are, you must Licensed for the State of IL : LSW, LCSW, LPC, LCPC and LPNReliable transportation and Auto insuranceAbility to work M-F, 8 AM-5PM travel in the field.Bachelor Degree in Social Services or related field required.Previous experience working with the Medicaid Waiver Population preferredExperience with MCO’s a plusPreferred, LTSS/HCBS waiver experiencePreferred, Care Coordination experiencePlease note Salary range is based on years of experience and licensure.AltaStaff is an Equal Employment Opportunity Employer.
